The 44 weeks of Professional Practice Education (PPE) experiences are integrated throughout the curriculum:
- DPT 6318 Professional Practice Education 1: 6 weeks at the end of the 3rd semester during the 2nd academic year (PT2)
- DPT 7428 Professional Practice Education 2: 8 weeks at the beginning of the 6th semester during the 3rd academic year (PT3)
- DPT 8818 Professional Practice Education 3: 16 weeks during the 7th semester during the 4th academic year (PT4)
- DPT 8728 Emerging Roles in Physical Therapy Practice 4: Individualized 14-week experience during the 8th and last semester of the 4th academic year (PT4)

Clinical Site Information Form (CSIF): If you have a clinical affiliation agreement to accept students then please fill out the form in your prefered format.
CSIF Web
Our school will be utilizing CSIF Web this year to help manage and keep track of our clinical sites. We are asking our clinical sites to fill out the CSIF online at https://csifweb.amsapps.com. We realize this may take some time initially but save time in the future. If you prefer to not use CSIF web then you may access the CSIF form via the Word document below. The completed form allows students to efficiently learn about your clinic and aids in their decision with matching. It also allows for our communication to be more effective and efficient while having a better idea of clinical environment with matching and evaluation of the student in the clinic.
CSIF Web will allow clinical sites to have one place to fill out their CSIF for all programs they work with, even if they are not using CSIF Web. Once you have invested time in completing the CSIF, it will be stored safely in CSIF Web and each year, (or more often if you like), you can just update your data, and schools using CSIF Web will automatically have access to that information! If schools are not using CSIF Web, you can send them a PDF of your completed CSIF by downloading it from the ‘Sites’ tab in CPI Web.
To login to CSIF Web, go to https://csifweb.amsapps.com. If you have logged into CPI Web before, your login information is the same for CSIF Web.
If you have not logged in to CSIF Web or CPI Web, your username for CSIF Web is your email address. To create a password, please use the ‘I forgot or do not have a password’ link on the homepage.
